Bridges/GCR
What is Bridges?
Bridges is a joint program between Acute Geriatric Services and Home Care offering supportive services to medically complex, frail, elderly people wishing to remain in their own home but who are at risk of institutionalization.
This program:
- offers restorative and rehabilitative services that facilitate a client’s ability to remain as active and independent as possible
- aims to support individuals in maintaining or improving their well being
- assists in reducing caregiver stress
How Bridges works:
- A client may attend the program 1-5 days per week Monday through Friday.
- The program operates from 9:30am – 3:30pm.
- Individual attendance may vary within the hours of operation to meet the need of the client, caregiver and the availability of services.
What is GCR?
Geriatric Community Rehabilitation (GCR) is a short-term outpatient program specializing in client assessment and rehabilitation. One of the goals of GCR is to support the frail elderly and their families in their desire to remain at home and in the community.
How GCR works:
- The program involves a health care team approach that works closely with the client, family physician and community agencies such as Home Care.
- Attendance will be scheduled for two days per week.
- Clients can choose morning or afternoon programs dependant on availability of space.
