Post Acute Rehab Program (P.A.R.P.) - Pharmacist

Pharmacist 1 Regular Full Time FTE: 1.0FTE Department: Rehabilitation at St. Michael's Health Centre Hours of work: Days (7.75 hour shifts; Monday-Friday; some flexibility required) Responsibilities: Under general supervision, the Pharmacist ensures the safe and efficient preparation, distribution and utilization of pharmaceuticals and related materials to patients. The Pharmacist provides pharmaceutical care services, including drug therapy monitoring, outcome evaluation and the review of drug utilization trends to ensure safe, effective, rational, legal and economic use of drugs. The Pharmacist also provides education to hospital health professionals, residents and/or care providers. Qualifications: • Bachelor of Science Degree in Pharmacy required. • Licensed or eligible for licensure with the Alberta College of Pharmacists required. • Completion of an accredited Canadian Hospital Pharmacy Residency Program an asset. • Prior clinical experience with patient pharmacotherapy monitoring, multidisciplinary rounds and education programs is desirable. • Previous experience working in a Rehabilitation environment is preferred.
